HSHS St. Anthony’s Memorial Hospital – Effingham, Illinois

HSHS St. Anthony’s Memorial Hospital convened a multi-disciplinary team to address how they  could improve sepsis bundle compliance and decrease morbidity and mortality. Today their bundle compliance has increased from as low as 37% to 80% and their mortality rate has decreased from 15% to 3%.

Results of St. Anthony’s Memorial successes and any opportunities for improvement are shared with their sepsis team members and the individual clinicians caring for the patient. They continue to monitor bundle compliance and mortality rates on a monthly basis and are ready to spread this intervention to another hospital in the state!

Their team is in the process of putting together a toolkit of order sets, their tick sheet and their algorithm. Some keys aspect of their process include:

  • Utilization of a Pareto chart to help identify the individual metrics of the sepsis bundle which need to be addressed.
  • Development of a sepsis treatment algorithm in early 2016, which has been instrumental in assisting their clinicians with appropriate treatment.
  • Sepsis patients are followed with a tick sheet that allows for real-time intervention. Their clinicians (both physicians and bedside nursing staff) assisted with the development of the algorithm and tick sheet. This was key to their success.
  • Monthly review of outcomes with their sepsis team helps drive improvements, especially around those metrics where there is opportunity.
  • Integration of Sepsis order sets. Including: ED and inpatient, for adult, pediatric and neonatal patients.

Spread Partner Criteria for HSHS St. Anthony’s Memorial:

Must Have:

  • A physician champion: ED and house-wide
  • A strong abstractor
  • An organization that is willing to take on house-wide deployment of this process
  • The ability to integrate key aspects of this process into their EHR and to modify their EHR after implementation
  • A Lab with a good turnaround time
  • The ability to identify patients in real time (real time alert system)
  • The ability to manage all of the bundle components (i.e. pressors)
  • The culture and willingness to give and take feedback across the multidisciplinary team
  • Multidisciplinary team: Pharmacy, IT, Lab, Quality, ED Manager, ICU Manager and Physician Champions 

Should Have:

  • Nursing support
  • An engaged CMO
  • Willing to host or travel in order to work with the St. Anthony’s Memorial team (Effingham, Illinois)

Could Have:

  • Press Ganey
  • Theradoc
  • Current bundle compliance of 30-50%
  • Knowledge of your Sepsis Mortality Rate
  • A Sepsis Team already in place
